1. The physical examination clinic services are scheduled for 8:30-10:30 am and 13:30-15:30 pm, Monday through Friday. Services are available by appointment only.
2. On the first visit to the NCKU Hospital for a medical service, applicants who have completed a designated online form for the first clinic consultation should go to the registration counter to produce their medical record number before they proceed to the physical examination counter.
3. Applicants should register for the outpatient clinic for a physical examination by appointment in order to receive a medical report. An application for a copy of a Chinese medical report requires a 2-inch photo and a processing fee of NT$30.
4. Please note that new students of NCKU who fail to receive a physical examination during the scheduled preferential period should register for the clinic designated for NCKU Physical Examination for New Students by appointment starting from September 1.
5. Please register for the travel clinic if you apply for an International Certificate of Vaccination or Prophylaxis (ICVP, the Yellow Book) or seek consultation for overseas study or travel.

NCKU Physical Examination for New Students

1. Please register for an outpatient clinic number and pay the registration fee during the service hours of 08:13-10:30 am and13:03-15:30 at the referral center on the first floor of the outpatient clinic building before you proceed to the physical examination counter on the second floor. Please be reminded that services are available only during the scheduled hours.
2. Applicants can search for their physical examination report online on the Web site of NCKU Physical Examination for New Students at https://service.hosp.ncku.edu.tw/PhoneReg/OHM/OHMNckuStudentReport.aspx, starting from 2022.
3. Applicants who are not fasting on the date scheduled for the physical examination should complete the examination according to the laboratory testing form issued by the physical examination counter within two weeks from the original date of the examination.
4. Female applicants who undergo a physical examination during their menstrual period will receive a urine test tube on the day of examination. Starting two days after the end of the menstrual period, applicants are allowed to submit the urine-collected tube to the blood drawing counter (to be completed within two weeks from the date of the physical examination).
